Now Accepting Nominations for the 2025 MHHA Hall of Fame
The Hall of Fame was inaugurated to honor those individuals who have excelled in the home care and hospice profession; those who bring honor and recognition to the Michigan HomeCare & Hospice Association (MHHA); and those who inspire and mentor individuals and students to consider or continue their careers in home care and hospice. The winners of this award will join a select group of past inductees who, through their career achievements, exemplify the very best by continually bringing credit to their profession.
ELIGIBILITY: Nominees for this award have been or currently are employed or involved in one or more home care and hospice industry segments including Home Health, Hospice, Palliative Care, Private Duty, Home Medical Equipment, or Pharmacy Infusion for a combined minimum of ten years at the time of nomination. Nominees must have made significant contributions to Michigan’s home care and hospice industry. Retired or deceased candidates are also eligible. Members of the MHHA Board of Directors will approve the selection of new inductees and are ineligible to be awarded into the Hall of Fame while actively serving as a voting member of the Board.
Requirements for Nominations:
Deadline for nominations is February 21, 2025. MHHA membership is not a required criterion for nominators or nominees. It is the responsibility of the Nominator to compile a nominations package to demonstrate the qualifications and credentials of the candidate. Nominations which are submitted will be kept on file for three years to be considered for possible selection. After three years, a candidate must be re-nominated to receive further consideration.
Nomination Package shall include two required criteria. Optional criteria are encouraged to be included with the nomination package.

Cover letter explaining why the candidate is deserving of Hall of Fame recognition. This cover letter should detail the nominee’s home care and hospice professional experience describing the service made to the Home Care & Hospice Industry and any achievements with current and/or past organizations. (required).
Submit a photo taken in the past five years. (optional)
Additional letters of recommendation. (optional)
Contributions to voluntary organizations or causes. (optional)
Participation in civic and community affairs. (optional)
Other materials to show the accomplishments of the nominee. (optional), i.e. magazine or newspaper articles, awards received and other contributions to the persons profession, organization, or society in general. (optional)
Nominations must be postmarked or received at the MHHA Offices (2140 University Park Dr., Suite 220, Okemos, MI 48864) by February 21, 2025.
Only the MHHA staff and Board of Directors are aware of your nomination package unless the person nominated is selected for the Hall of Fame award. Nominators of Hall of Fame inductees will be notified individually. It is the privilege of the nominator to notify the recipient of this honor. The inductees to the Hall of Fame are given advance notice before being recognized at the MHHA Annual Awards Banquet held May 7, 2025.
